[DECtalk] L&H TTS3000 sapi4 with american english voices and an interesting version of truvoice

Warning: After installing and switching NVDA to SAPI4, the default voice is
a German female. That was the case on my system anyway. You will eventually
get to English after arrowing down through Japanese and Dutch.
