Reaper i believe as part of the SWS Extensions have Smart Markers. These are markers that you can assign actions to so whenever the play head crosses that marker the action is triggered. I believe they can also be used to trigger and or send midi notes/events. Only thing i can think of that may come close to what you want to do.
