An "item" is one section of media in a track. However, an item can have
one or more "takes". The concept is exactly what you'd expect; i.e.
multiple "takes" or attempts at a specific section. So, each take
consumes the same section of time, but you can switch between the takes
at will. If you split an item with multiple takes, both new items
(before and after the split) will also have the same number of takes.
Both items and takes have separate volume controls. If you tweak the
volume for an item, that will affect all takes, but if you tweak the
volume for a specific take, only that take will be affected. In
practice, this could be useful if, for example, a vocalist backed off
the mic when recording a particular take, but was closer for all other
takes.
I've only been using REAPER for a few months, so more experienced users
may wish to correct anything here. :)
