[RWP] Extran Items

Guys,
Why does reaper sometimes create more items that  one would expect?  I am 
pretty sure I  placed the cursor somewhere, and pressed s, only to hear 
reaper say that 3 items were added.  Does that work across tracks when you 
do that?
But also, I think I have recorded a single session of audio, only to come 
back and find that the sessions actually consists of more than one item. 
Why is that?
I thought an item was a chunk of audio recorded in a single take.
Anyway, any ideas most welcome.
