Hi Stephan,
There is no manual yet for Reaper with ReaAccess.
Feel free to ask questions here.
1. Install the proper Reaper for your operating system; 32 for XP; 64
for 64 bit Windows 7.
2. Then open and close Reaper.
3. Then install ReaAccess.
4. Open Reaper and press ctrl+P for preferences.
5. Arrow up to Audio and configure your audio.
Hopefully, you have bought a professional audio and midi interface card,
otherwise latency will be very large with the built-in RealTech sound chip.
After you have arrowed to Audio, or Device; or Midi Device; tab down to
find the fields to configure for that title.
6. After configuring audio and audio device, arrow down to Midi Device
and enable your midi interface, the professional sound card or RealTech
if you have nothing else; plus enable your midi keyboard.
You need to get the word Enabled; instead of Disabled; after the name
of your midi interface and after your midi keyboard.
Focus onto the device name; then route to the number pad cursor,
whatever that move is called in your particular screenreader.
Find the name of the device at the numberPad and right click on it.
The Configure dialog will pop up.
7. Left click on Configure, and place a check mark with the spacebar
onto use as midi device, and also on use for midi controller if your
midi keyboard has a fader; or other controls that transmit midi messages.
8. Tab down to OK and press enter.
You will be taken back to track view.
For the ReaAccess shortcuts press F12 for Learning mode.
In Learning Mode you can read all the shortcuts without the shortcuts
carrying out their function.
Press F12 again to return to normal mode.
Press alt+I to insert a media file, like mp3 or midi file.
Arrow down once and you get Insert Soft Synth on New Track.
You can then browse to where Reaper keeps its bundled effects plugIns
and synths and press enter on a plug and it will be inserted.

Hope this gets you going.
