Here are my initial findings.
I used sonar, but that shoudn't matter anything made good for reaper will work out for sonar the vst ui's display a little differently with offsets and such but not worlds apart.
A few automatible parameters showed up in the synth rack,
didn't see anything in the ui itself except the default preset.
Was able to click on it and get a preset menu.
Some graphics which the screen-reader identified, but when I scanned them, no tool tips.
For this round I used window-eyes, I can try with nvda and jaws too.
I tend to like to use window-eyes when looking around new apps since it has the most useable review cursor, though nvda's object navigation is very good too.
Just a slight difference in design.

Thanks for the welcome Goldfinga,
As Goldfinga says I've been working on Poise to improve it's
accessibility. I've made a couple small changes already based on couple
suggestions. At the moment I'm working on labeling the graphics so the
GUI can be used with screen readers.
