Hi David,
When you save your current project, Reaper saves all your audio and midi.
Same as Sonar, just go to save project in the menus, name your current 
project and save it.
As for the IO dialog, first arrow to a track to select, then press the 
letter I to enter the IO Routing dialog.
The settings there are specific to the track you have selected.
Those send, return and hardware boxes are empty, waiting for you to make 
sselections in them.
In those boxes you assign sends and returns from tracks to tracks.
In reaper, audio and midi are treated the same, anything can go on a 
track,midi and audio on the same track if you want.
Instead of separate effects bins, you just put an effect onto a track.
For instance, put a soft synth onto a track; maybe put a reverb onto 
another track; then go into the IO dialog and create a send from the 
synthesizer track to the reverb track, or a return from the synthesizer 
track to the reverb track..
Think of tracks with send arrows pointing to other tracks, or returns 
pointing back, to get any routing scheme you need.
I myself like to keep each synth and each effects plug on its own track, 
for clarity, but other people work more complicated than that.
Soon it will make sense to you.
Have you inserted a soft synth?
You need to have soft synths in a particular folder, like 
C:>Steinberg>VSTPlugins, or wherever you prefer, then fill in the entire 
path in preferences VST, so that Reaper can find your plugs.
Be sure to click the apply and okay buttons after making changes in 
options preferences.
Then; to insert a synth on its own track,  press ctrl+insert, the middle 
insert key up above the middle arrow keys, then use your tab and arrow 
keys to find the VST folder, click enter on a synth and it will be 
placed on a track.
That track is the master track for that synth.

While arrowed to a track, press your tab key to get track options, like 
the  armed for recording state, its automation selections, etcetera.
Make selections with arrow keys and sometimes the spacebar.
Be sure to press enter to save your settings and return to the track view.

I don't know System Access, but I've looked at Reaper's simple 
procedures in Jaws, Window-Eyes and NVDA, and it all appears pretty much 
the same.
You did select System Access as a supported screenreader when installing 
ReaAccess, didn't you?
Open and close Reaper once, before installing ReaAccess, or you will get 
an error message.
Please excuse if you're way beyond all this basic stuff.
