really?
I'm running reaper version 3.76 and control shift M turns the met on and off all the time.
On both my acer netbook and the dell desktop.
I do find though that if you change the metronome click to straight quarters, so that the frequency is the same for all four beats instead of having the 1 as a different frequency, when I close reaper and reopen it, the met settings go back to default. They don't seem to save when you change them.

Speaking of that, with Reaper 3.76 anyway, haven't done anything with 4 yet under either Mac or Windows, control shift m almost never works to turn the metronome on and off. I always have to do it through options.

Go to metronome and preroll settings under options. You can set preroll, tempo, time signature, and type of sounds used, as well as the frequency of the clicks and so on.
Unfortunately, no midi out option. The metronome in reaper is audio bassed only.
