Because the document is longer than 50 pages and Reader defaults to
allowing 50 pages before it goes into page mode. This is done due to the
amount of time it takes ot render large documents.

Yes. You can do it just for the current session using Document menu ->
Change Accessibility Reading Options (ctrl+shift+5) or you can do it
permanently on the Reading page of the Preferences or via Document menu
-> Accessibility Setup Assistant.

This is why NVDA ignores Reader's idea of line breaks. Unfortunately,
this causes the reverse problem in some documents. Reader's handling of
line breaks in terms of accessibility really sucks. I've reported this
to Adobe in the past, but no joy on a solution yet.
